Our Specialty Court and Behavioral Health Divisions have a team of Case Managers who work with each Participant. Our goal is to ensure that each Participant is connected to the resources they need in order to be successful, not only while they are completing our programs, but in their recovery journeys after the program.
Each Case Manager is assigned a specific court(s). With each Participant, the Case Managers complete an assessment. The Case Managers then assist the Participant with any resources they may need. For example, the Case Manager may help a Participant apply for benefits, housing, or help them find a job. These needs are reassessed at each Phase, and the Case Manager will work with the Participants throughout the program should needs arise.
Further, each court has five phases. In phases 2-4, the Case Manager works with the Participant formulating a Case Plan. The Case Plans are designed to help the Participants set and complete goals while they are in our programs. The goals should be focused on the Participant’s highest “risk areas” as were determined from the assessment. Finally, in the final phase, the Case Manager completes an Aftercare Plan with each Participant. We would like each graduate from our programs to have all the support and resources they will need for their continuing journey of recovery.
